i want to create modify form from list item that contain 3 choices(insert,update,delete

if I choose any one from list the departments block will appear to make insert in it or update or delete .


- In delete/update case the department will choose from list of values(lov).
- In insert case the new department number will automatic generate with the same sequence numbers in departments table.
- In the end of transaction the alert will be appear to notification us.
