I Need Help In That Parameter

i need help i have tow forms the first form contains 2 text item of date
and 2 textitem of char i want take that values in parameter and when i open the other form the defaut_where of the block ' drivers' change to that values and making execute_query
this block it's fields (start_date date) and (end_date date) and (dr_name varcha2(30)) and (status_or varchar2(10)) how can i make that
